Oxford University, Brookes
University and other academic
institutions in the City.
Your education establishments may
have arrangements with certain
practices, but you are free to
register where ever you choose. It
may be more convenient to register
with us near to your accommodation
rather than your college. Please
note that you do need to live in our
catchment area to
register.
You may find it convenient that you
can download the registration form
and questionnaire (see the links
below). You can
complete this at your leisure and
bring it with you at the time of
registration or post or email it to
us. However, you will not be
registered until you have come to
the surgery.
